CLIENTS WITH INSURANCE COVERAGE

Private insurance policies vary in coverage. We recommend that you contact your insurance company to determine what cost, if any, it will reimburse for outpatient psychotherapy services.

Co-pays are due at the time of service.

CLIENTS WITH MEDICAL ASSISTANCE

Clients with managed care coverage for their Medicaid coverage will be eligible for treatment upon authorization by their Managed Care/Insurance Carrier.

CLIENTS WITHOUT INSURANCE COVERAGE OF ANY TYPE

The fee for services is determined using a sliding scale.

To determine your liability, a copy of a recent pay stub or income tax return is required.

Should there be a change in your financial status or insurance status, please notify the receptionist immediately.

You may be eligible for Medical Assistance based on your income and expenses. Medicaid could assist you in receiving the essential medical care for yourself and your family.

Payment is due at the time of service.

The usual and customary cost of therapeutic services at Phoenix is as follows:

Intake/Evaluation   $ 200.00

Individual Therapy  $ 110.00

Family Therapy   $ 110.00

Group Therapy   $ 60.00 per group session

IOP  $ 150.00 per day
